Fingerprint Clearance Card for Nurses

The actual card with your fingerprints that you provided the Arizona Board of Nursing with your application is not an Arizona Fingerprint Clearance Card. You must apply for a fingerprint clearance card with the Arizona Department of Public Safety in order to obtain a fingerprint clearance card.

However, many health care organizations in Arizona require their workers to obtain a Level One Fingerprint Clearance Card. If you are a nurse and are charged or convicted of a crime while you have a fingerprint clearance card it will likely be suspended. If your card is suspended you will be unable to provide care if your facility requires you to have the card.

The only way to get the card back from suspension is to apply for a good cause exception with the Arizona Board of Fingerprinting.
